Investigators have not completed their inquiry into the official cause of the accident, but they were able to tell sources that the worker had been operating a forklift prior to the accident and had entered onto an area of the floor that was apparently not fully supported. The excess weight caused the machine to crash through the floor and fall approximately 15 feet onto a basement floor. The fall, accompanied by debris that toppled down with the forklift, left the operator of the machine with fatal injuries.

Magezi, an employee of Tree Are Us, suffered a traumatic head injury and was rushed to Memorial Hospital where doctors pronounced her dead. An autopsy will take place next week to confirm the cause of death.
The owner of Trees Are Us, Magezi’s stepfather, who was not present at the time of the deadly worker accident, is under investigation by the Occupational Safety and Health Administration. Magezi was not wearing a hard hat at the time of the Indiana on-the-job accident.

Indiana State Police report that a utility worker was killed by a coal truck on Route 22 in Armstrong Township, Indiana.
Reports say that 48-year-old Robert Conner of Warsaw Township in Jefferson County, IN, was struck by the coal truck while locating a utility line near a construction zone. Conncer was standing next to his utility truck on the east side of the highway where the road narrows from two lanes to one. Allegedly, 59-year-old Carl White of Avonmore, Indiana was driving a coal truck eastbound past this point when he hit and killed Conner.
Police, firefighters, and EMS responded to the scene and rushed Conner to Indiana Regional Medical Center where he was pronounced dead. The Indiana construction site accident is still under investigation. The Memorial Medical Center in Johnstown will perform an autopsy on Conner this week.
Iron worker Stanley Roberts, 55, was fatally injured Wednesday after falling more than 50 feet from his metal construction lift that toppled over for reasons that remain unknown. There has been speculation that Mr. Roberts' was strapped in a faulty harness, which contributed to his fatal injuries.
