The Cook County medical examiner’s office has ruled the death of 53-year-old Jim Tyree an accident. While the Sun-Times Chairman and Chicago personality had been diagnosed with stomach cancer last year, and while he had been unwell, his actual death was a preventable medical error.
The autopsy of the man revealed that he died from an air embolism – air bubbles in the blood stream probably caused when a dialysis catheter was removed. Tyree died just minutes after the catheter was removed. Friends who saw the man said that he was absolutely alert the day of his death. The accident took place as he was being treated at the University of Chicago Medical Center.
The hospital, of which Tyree was a board member, released a statement that they are deeply saddened by the businessman’s passing, but did not release any details about the manner of his death and whether it was caused by a medical error.
Air embolisms are considered a preventable medical mistake – and one that should be prevented when nurses notice air in IV bags or change IV bags. It is not know if Tyree’s family will file a medical malpractice lawsuit against the hospital.
Tyree, whose death was mourned by the Chicago business world as well as President Obama, was known for many accomplishments during his life, including significant charitable work. He was suffering from stomach cancer and pneumonia during his hospital visit, but was looking forward to returning home in the days before the deadly medical mistake.
