In Gary, Indiana, a construction worker's fatal fall has led to the discovery of a number of construction site safety violations and two hefty fines for industry behemoth the US Steel Corporation.
On June 6, 54-year-old Alexander Santoyo, of Valparaiso, Indiana, was killed when he fell off of an overhead crane at Gary Works. According to the Lake County Coroner's Office, the man died of blunt force trauma at the scene of the construction accident at approximately 4:35 in the afternoon.
Following the Indiana construction worker death, the Indiana Occupational Safety and Health Administration (IOSHA) investigated the fatal industrial accident on several occasions in search of the cause of the on-the-job accident as well as any US Steel Copr safety violations. First, the IOSHA discovered that fall protection was not being used by workers who were working on overhead cranes - even at deadly heights of 80 to 90 feet. Secondly serious ladder violations were found, in which ladders had rungs less than seven inches from the back of the crane frame, making it difficult for workers to support their weight while climbing.
These two serious safety violations led to two hefty fines totaling $10,000. Both safety violations were considered serious enough to case serious worker injuries or even worker death. A spokesperson from US Steel Corp told the media that they would pay the fines without objection and correct the safety issues discovered during the investigation.
